Unable to palpate rest of abdomen due to client guarding of abdomen. Client reports being constipated for last month . Vital signs: temperature, 101.8°F (38.8°C) ; heart rate, 102 beats/min ; respiratory rate, 18 breaths/min; blood pressure, 108/64 mm Hg. Body mass index (BMI) 32 .
