Diagnosis: bipolar disorder DOB: 6/5/1986: 8094539 Room: 107 Day 11500 Avery Vergara, 38 - year - old male, is brought to the mental health facility by his spouse who states he has been displaying manifestations of mania over the past few weeks. The spouse states that Avery was diagnosed with bipolar disorder two years ago, after experiencing several episodes of mania that were followed by stages of major depression. The spouse believes her husband has stopped taking their prescribed medications. She states, He never sleeps, hasnt eaten in days, and just talks all the time. He comes up with all of these ideas about writing and producing screenplays. I dont remember the last time he had a shower. And then today I got a call from his boss at work. Apparently, he just walked off the job with no notice to anyone. No one could find him, and he wouldnt answer his cell phone. It took hours, but I finally found him at home curled up in a ball on the bed, asleep. He had walked over ten miles to get home.According to the spouse, Averys bipolar episodes follow a familiar pattern of mania for a couple of weeks, then he gets irritable and anxious, and then goes into a terrible depression.
